Immigration document

Green card, visa, EAD, I-94 or other proof of status. Replaces the SSN in most states.

Driver's License (or state ID)

Photo ID, legible and within its validity period.

Proof of income

Pay stub, job offer letter, statement or self-employment income declaration.

U.S. address

Utility bill, internet bill or lease with your name and current address.

SSN (only NJ, PA and VA)

When to enroll

Enrollment periods

There are two moments to join a plan. Knowing which one is yours keeps you from missing the deadline.

Life change event (SEP)

Changed jobs, got married, had a baby, lost a previous plan or moved to a new state? That opens a special enrollment window outside the regular period. Some states (like Pennsylvania) require proof of the event.
